       50 years old female patient  came with an chief complaint of High grade feaver since 10 days
HISTORY OF PRESENT ILLNESS 
         50 years old female patient who is daily labor by occupation ,was apparently asymptomatic 10 days ago and then she developed continuous high grade fever with no history of chills
      Feaver associated with cough and cold then She went to local hospital and treatment taken ,subsided.And fever reappeared 3 days ago high grade fever associated with chills 
HISTORY OF PAST ILLNESS 
    Not K/C/O Diabetes
    Not K/C/OHypertension
,asthma,Epilepsy 
    Thyroid since 12years
PERSONAL HISTORY 
    Status : Married 
    Occupation: labor
    Diet : Mixed
    Appetite  : Normal
    Bladder movements : Regular 
    Bowel movements  : Regular 
    No issue of alcohol and Smoking intake
Family History 
    No history of diabetes, hypertension,asthma ,stroke
General Examination 
  Patient is conscious, coherent, cooperative  
No history of cyanosis, clubbing lymphadenopathy, pallor, edema
Physical status:Moderately built
Adequate hydration

Systemic Examination 
    Cardiovascular system
    Cardiac sounds:S1 S2+
     Respiratory system 
     Dyspnoea - No 
     Wheeze - No

    Dengue fever with thrombocytopenia
